Dr. Diksha Gupta, our board-certified Family Medicine physician, explains what high triglyceride levels actually mean for your heart and metabolic health, how triglycerides differ from cholesterol, what counts as a dangerous triglyceride level, the important link between very high triglycerides and pancreatitis, which foods raise or lower them, and how to lower triglycerides naturally through diet, exercise, and lifestyle changes — including how quickly you can expect to see results.
If you’ve recently had routine blood work and your doctor told you that you have high triglycerides, you may be wondering what that actually means. Are triglycerides the same thing as cholesterol? Is having high triglycerides dangerous? And most importantly, what can you do to lower them?
As a primary care physician, I see elevated triglycerides frequently on routine cholesterol testing. The good news is that triglycerides are often very responsive to lifestyle changes. For many people, improving their diet, increasing physical activity, reducing alcohol, and addressing other health conditions can make a meaningful difference.
Understanding your numbers is the first step toward better triglyceride management. Let’s take a closer look at what triglycerides are, what causes them to rise, and what you can do to bring them down.
What Are Triglycerides?
Triglycerides are a type of fat found in your blood. When you eat more calories than your body needs—particularly from carbohydrates, fats, and alcohol—your body can convert those extra calories into triglycerides and store them as fat.
Triglycerides aren’t inherently bad. Your body needs them for energy. The concern is when levels remain elevated over time.
Triglycerides are typically measured as part of a lipid panel test, which may also include:
- Total cholesterol
- LDL cholesterol (“bad” cholesterol)
- HDL cholesterol (“good” cholesterol)
- Triglycerides
This is why triglycerides are often discussed alongside cholesterol during routine cholesterol screening.
Triglycerides vs. Cholesterol: What’s the Difference?
Patients frequently ask about triglycerides vs cholesterol, and although they’re related, they aren’t the same thing.
Cholesterol is a waxy substance your body uses to build cells and produce certain hormones. Triglycerides are primarily used to store excess energy.
Both can contribute to cardiovascular risk when abnormal, but they are influenced by somewhat different factors.
For example, triglycerides are particularly sensitive to:
- Excess sugar and refined carbohydrates
- Alcohol
- Excess calorie intake
- Obesity
- Poorly controlled diabetes
- Certain medications
- Some genetic conditions
That’s why improving your triglycerides may require a slightly different approach than simply focusing on your cholesterol.
What Are Normal Triglyceride Levels?
A triglyceride blood test is reported in milligrams per deciliter (mg/dL).
For adults, levels are generally categorized as:
- Normal: Less than 150 mg/dL
- Borderline high: 150–199 mg/dL
- High: 200–499 mg/dL
- Very high: 500 mg/dL or higher
Your physician will interpret your results in the context of your overall health, including your cholesterol levels, blood pressure, blood sugar, weight, medications, and family history.
A single abnormal result doesn’t necessarily mean you have a serious health problem. What matters is understanding why the number is elevated and whether it remains high over time.
Are High Triglycerides Dangerous?
One of the most common questions I hear is, “Are high triglycerides dangerous?” The answer depends on how elevated they are and whether you have other health conditions.
Mild to moderately elevated triglycerides may not cause immediate symptoms, but when they remain high over time, they can be associated with an increased risk of heart disease, heart attack, and stroke, particularly when combined with other risk factors such as high LDL cholesterol, diabetes, high blood pressure, smoking, or obesity.
Very high triglycerides can cause a different and potentially serious problem: acute pancreatitis.
High Triglycerides and Pancreatitis
Pancreatitis is inflammation of the pancreas, an organ located behind the stomach that plays an important role in digestion and blood sugar regulation.
The risk of pancreatitis begins to increase when triglycerides are very high, particularly above 500 mg/dL, and becomes substantially higher when levels reach 1,000 mg/dL or more.
Symptoms of pancreatitis can include:
- Sudden or severe upper abdominal pain
- Pain that may radiate to the back
- Nausea and vomiting
- Fever
- Abdominal tenderness
- A rapid heartbeat
If you have very high triglycerides and develop sudden, severe abdominal pain, particularly with nausea or vomiting, you should seek urgent medical attention. Acute pancreatitis can require hospitalization and treatment.
The good news is that pancreatitis related to high triglycerides is often preventable. If your triglycerides are significantly elevated, working with your healthcare provider to identify the cause and lower your levels is important.
Other Health Problems Associated With High Triglycerides
High triglycerides are also commonly seen alongside other metabolic health problems, including:
- Type 2 diabetes and insulin resistance
- Obesity
- Metabolic syndrome
- Fatty liver disease
- High blood pressure
- Abnormal cholesterol levels
- Increased cardiovascular risk
In many cases, these conditions are interconnected. For example, someone with insulin resistance may have high triglycerides, low HDL cholesterol, increased abdominal weight, and fatty liver disease at the same time.
This is why I don’t look at your triglyceride number in isolation. Your triglycerides are one piece of the larger picture of your metabolic and cardiovascular health.
Do High Triglycerides Cause Symptoms?
Unfortunately, high triglycerides symptoms are usually nonexistent.
Most people with elevated triglycerides feel completely normal.
This is why regular triglyceride testing and cholesterol screening are so important. You cannot reliably tell whether your triglycerides are elevated based on how you feel.
Occasionally, people with extremely high triglycerides may develop fatty deposits under the skin or around the eyes, but this is uncommon.
What Causes High Triglycerides?
There are many possible reasons for elevated triglycerides.
Common contributors include:
- Eating a diet high in sugar and refined carbohydrates
- Excess alcohol consumption
- Being overweight or obese
- Physical inactivity
- Poorly controlled diabetes
- Hypothyroidism
- Kidney disease
- Certain medications
- Genetic conditions
Sometimes several factors contribute at once.
For example, a patient may have a family tendency toward elevated triglycerides, but diet, alcohol, and inactivity may push the numbers even higher.
How to Lower Triglycerides
The question patients ask most often is, “How to lower triglycerides?”
For many people, the answer starts with lifestyle.
1. Reduce Added Sugar
One of the most effective changes you can make is reducing added sugar.
Pay attention to:
- Soda
- Sweet tea
- Energy drinks
- Desserts
- Candy
- Sweetened coffee drinks
- Sugary cereals
- Baked goods
Excess sugar can be converted into triglycerides, particularly when consumed regularly.
2. Cut Back on Refined Carbohydrates
You don’t necessarily need to eliminate carbohydrates.
Instead, focus on replacing highly processed carbohydrates with higher-fiber options.
Reduce:
- White bread
- White rice
- Pastries
- Chips
- Crackers
- Sugary cereals
Choose more often:
- Whole grains
- Beans
- Vegetables
- Lentils
- High-fiber foods
3. Limit Alcohol
Alcohol can have a significant effect on triglycerides.
For some people, even moderate alcohol consumption can cause levels to rise. If your triglycerides are significantly elevated, your physician may recommend avoiding alcohol altogether.
4. Choose Healthier Fats
Not all dietary fat is the same.
Choose foods containing healthier unsaturated fats, such as:
- Avocado
- Olive oil
- Nuts
- Seeds
- Fatty fish
Limit foods high in saturated and trans fats.
Foods That Lower Triglycerides
There isn’t one magical food that will lower triglycerides, but building a heart-healthy eating pattern can make a significant difference.
Some of the best foods for high triglycerides include:
- Salmon
- Sardines
- Tuna
- Oatmeal
- Beans
- Lentils
- Vegetables
- Berries
- Apples
- Nuts
- Seeds
- Whole grains
Fatty fish are particularly helpful because they contain omega-3 fatty acids.
Foods to Avoid With High Triglycerides
If you’re looking for a practical diet to lower triglycerides, focus less on eliminating entire food groups and more on reducing foods that are likely to raise your levels.
Foods to limit include:
- Sugary beverages
- Candy
- Desserts
- Sweetened cereals
- White bread
- Large portions of refined carbohydrates
- Fried foods
- Processed meats
- Excess alcohol
Portion size matters too. Even healthy foods can contribute to weight gain if consumed in excess.
Exercise for High Triglycerides
Physical activity is another important part of triglyceride management.
You don’t have to become a marathon runner.
Start with activities you can consistently maintain:
- Walking
- Cycling
- Swimming
- Dancing
- Strength training
- Hiking
A reasonable goal for many adults is at least 150 minutes of moderate-intensity activity each week, along with muscle-strengthening activities.
Even adding a 20–30 minute walk to your daily routine can be a meaningful first step.
How Quickly Can You Lower Triglycerides?
Patients often ask, “How quickly can you lower triglycerides?”
The answer varies.
Because triglycerides can respond relatively quickly to changes in diet, alcohol intake, weight, and activity, some people see improvement within several weeks.
However, sustainable improvement takes time.
Your physician may repeat your lipid panel after a period of lifestyle changes to see how your numbers are responding.
When Is Medication Needed?
Lifestyle changes are an important part of high triglycerides treatment, but medication may be appropriate depending on your numbers and overall cardiovascular risk.
Treatment may include medications such as:
- Statins
- Prescription omega-3 fatty acids
- Fibrates
- Other cholesterol-lowering medications
If triglycerides are 500 mg/dL or higher, your physician may recommend more aggressive treatment to reduce the risk of pancreatitis. At very high levels, dietary changes, eliminating alcohol, treating underlying conditions such as diabetes, and sometimes prescription medication may be necessary. Treatment should be individualized based on your triglyceride level and overall health.
The Bottom Line
Having high triglycerides doesn’t mean you’ve failed at taking care of yourself, and it doesn’t mean you’re destined to develop heart disease. Triglycerides are influenced by genetics, diet, activity, weight, medical conditions, medications, and alcohol.
The encouraging news is that they can often improve substantially with consistent lifestyle changes for high triglycerides.
